THE PURPOSE OF THIS ROLE IS TO WORK AS A HOSPITAL PHARMACIST ON ALL PARTS OF THE PHARMACY SERVICE PATIENT CARE , OPERATIONS AND QUALITY ASSURANCE SERVICE.

Specific Responsibilities:
Patient care
1. Verify physician orders for appropriateness according to HMC policies and procedures
2. Gather required patient information.
3. Maintains and updates a patient profile with demographics, diagnosis, allergies, and current medications
4. Identify, prioritize and resolve drug related problems
5. Uses appropriate communication skills, and effectively communicates with patients, physicians, and nurses to
solve identified drug related problems
6. Provide patient education (e.g., counselling information and education on adherence issues, either verbal or
written).
7. Monitor medication effectiveness by review of the medical record and direct questioning of the patient or
their care
8. Monitors drug therapy to evaluate appropriateness of use, drug, dosage form, regimen, route, therapeutic
duplication, and drug interaction

Experience and Education:

Experience And Education:
- Bachelor Degree in Pharmacy from recognized program
- 2 years pharmacy experience
OR
1 year experience for Advance Pharmacy degree (APD) holders
OR
0 years experience for Qatari candidates and graduates from recognized Qatar based
universities / institutes

- Valid Primary license from the country of origin (if applicable) AND should be eligible for
license from the Licensing authority in the State of Qatar to practice Healthcare
Profession.
